Yes, the Affidavit must be notarized.

Where do I go to get my Affidavit notarized?
You can visit our office between the hours of 8 A.M. - 5 P.M. Monday through Friday. You can also try a local bank or UPS Store.

What if I do not want to opt in to receiving email/text notifications?
You can still mail the Juror Questionnaire to our office or bring it with you on your service date. You will need to call 678-383-3333 the Friday prior to your service date AFTER 5 P.M. for instructions concerning your jury service.

What if I own my own business and cannot afford to attend jury service?
No one is exempt and/or deferred from jury service because of his/her job, race, color, religion, sex, national origin, sexual orientation, or economic status. In order to be deferred, you must fill out an Affidavit and submit it to the Jury Clerk.

What if I have an appointment on my day of service?
The juror must fill out the Affidavit, list their reasons for a request for deferment and return it to our office at your earliest convenience.
